Provide skilled physical therapy treatments to home health patients under the supervision of a licensed Physical Therapist. Educate patients and caregivers on mobility and safety while maintaining accurate regulatory documentation.
Provides skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist, following the established plan of care. Assists patients in improving mobility, function, and independence while delivering compassionate, patient-centered care. Key Responsibilities: Provide physical therapy treatments in the home health setting as delegated by the supervising Physical Therapist. Monitor patient progress, document treatments accurately, and communicate changes in patient status to the supervising therapist. Educate patients, families, and caregivers on exercises, mobility, safety, and home programs to support continued progress. Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care. Maintain timely, accurate documentation in compliance with regulatory, professional, and organizational standards. Participate in quality improvement initiatives and maintain compliance with all applicable policies, procedures, and state practice regulations. Promote positive patient experiences through professional, compassionate, and respectful care. Perform other duties as assigned. What You Offer Must be licensed or be eligible for licensure as a PTA in the state of Minnesota Valid driver’s license and auto insurance; ability to travel in all weather conditions Strong clinical, communication, and organizational skills Ability to work independently and maintain professionalism and confidentiality Communicate effectively in English, verbally and in writing. Pass a background study through the Minnesota Department of Human Services. Work Schedule Day Shift - 8am-4:30pm Casual or Roster Status
