Position Details

Provides skilled occupational therapy services to patients in their homes under the direction and supervision of a licensed Occupational Therapist, following the established plan of care. Helps patients improve independence, safety, and functional performance in daily activities while delivering compassionate, patient-centered care. Key Responsibilities: Provide occupational therapy interventions in the home health setting as delegated by the supervising Occupational Therapist. Document patient treatments, progress, and responses accurately and in a timely manner according to regulatory and organizational standards. Educate patients, families, and caregivers on home exercise programs, adaptive techniques, equipment use, and safety to support independence. Communicate patient progress and collaborate with the Occupational Therapist and interdisciplinary care team to coordinate care. Participate in quality improvement initiatives and support the development of safe, effective therapy services. Foster positive patient and family relationships through professional, compassionate, and respectful care. Maintain compliance with all applicable state regulations, professional standards, accreditation requirements, and organizational policies. Participate in community outreach and health promotion activities as appropriate. Perform other duties as assigned. What You Offer Must be licensed or be eligible for licensure as a COTA in the state of Minnesota Valid driver’s license and auto insurance; ability to travel in all weather conditions Strong clinical, communication, and organizational skills Ability to work independently and maintain professionalism and confidentiality Communicate effectively in English, verbally and in writing. Pass a background study through the Minnesota Department of Human Services. Work Schedule Day Shift - 8am-4:30pm Casual or Roster Status

Requirements
