The RN Care Manager plans and delivers individualized care to hospice patients through assessment, implementation, and evaluation. They collaborate with an interdisciplinary team and supervise caregivers to ensure high-quality patient-centered goals are met.
8:00am- 4:30pm participation in a holiday and weekend rotation. This is a field-based role that requires traveling to patient homes and care facilities throughout Marysville, Plain City, London, Jefferson, and surrounding areas.
We provide superior care for patients in families in homes/facilities. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.

Candidates must be a graduate of an accredited nursing program with a current Ohio RN license and CPR certification. Two years of acute care experience and CHPN certification are preferred, along with a valid driver's license and safe driving record.
