Aptive Resources, in partnership with Valor Healthcare, is hiring a Registered Nurse Care Manager to provide care coordination and chronic disease management within the PACT model at the proposed Yreka, CA CBOC, pending VA contract award.
Graduate of an ACEN- or CCNE-accredited school of professional nursing Current, full, active, unrestricted RN registration Current BLS certification Experience in care coordination and chronic disease management Must hold a current, full, active, and unrestricted license/certification (as applicable to the position) in California, or otherwise meet VA Qualification Standards for the position. Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
