Responsibilities

• Provide direct patient care to adults and geriatric residents per the agreed plan of care, including monitoring and reporting changes in condition.
• Administer medications and treatments in accordance with physician orders; perform 12-lead ECG/ECG as required.
• Document assessments, interventions, and patient responses in EPIC; communicate significant changes to the RN and MD promptly.
• Monitor vital signs, assess for complications, and participate in pressure ulcer prevention and wound care as indicated.
• Apply behavior management and de-escalation techniques; follow restraint protocols when applicable and maintain patient safety and dignity.
• Support activities of daily living and rehabilitation goals; collaborate with the interdisciplinary team (PT, Radiology, Social Services, Pharmacy) to coordinate care.

Requirements

• Active New York single-state LPN license required to submit
• Basic Life Support (BLS) certification
• EPIC charting experience required
• Minimum 1 year nursing experience; LTAC and Nursing Facility / Nursing Home experience required
• Prior charge experience preferred; travel experience required
• Availability to work 5x8 evenings; weekends every other weekend; and every other holiday as needed
• Ability to float within the documented scope of practice
• RTO requests of more than 10 days are not accepted; verify any time-off needs with the Account Manager prior to submission
• Locals policy: verify with Account Manager before submitting if local candidates are being considered
