Become a part of our caring community Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner (NP), or Physician Assistant (PA) to join our team at our clinic in beautiful and sunny Jacksonville, FL As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team.

This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

Your Role & Responsibilities You will provide comprehensive care to seniors following clinical standards.

You will collaborate daily with your care team to ensure coordinated, high-quality care.

You will manage referrals, hospital/SNF coordination, DME, and home health services.

You will document care with support from quality-based coders.

You will participate in on-call rotation and support growth plans at your center.

You will have experience with providing value-based care.

You will work on-site.

What We're Looking For Required: Master's degree in nursing with board certification.

Active, unrestricted NP, or PA license in the state of Florida.

Experience in value-based care, primary care or Medicare Advantage settings.

Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers Familiarity with HEDIS quality indicators and Medicare guidelines. #LI-FM1

Use your skills to make an impact Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan.

This incentive opportunity is based upon company and/or individual performance.

Requirements
