About the Role

Clarvida is seeking a CCFT Care Coordinator to provide therapeutic interventions, case coordination, and resource linkage services for children, adolescents, and families. In this role, you will work collaboratively with clients, caregivers, treatment teams, and community partners to support positive behavioral, emotional, and family outcomes. As a Care Coordinator , you will deliver individualized services that promote skill development, crisis stabilization, behavior modification, and family strengthening. This position involves community-based work, including client visits, coordination with external stakeholders, and advocacy to ensure clients receive the resources and support needed to achieve treatment goals. The ideal candidate is passionate about helping children and families, skilled in relationship-building, and committed to delivering quality, client-centered care.

Responsibilities

Conduct client assessments and intake activities as assigned Provide community-based services, including home visits and client transportation as appropriate Deliver Clarvida-approved therapeutic interventions, including: Coping skills development Behavior modification strategies Parenting skills training Crisis intervention services Collaborate with treatment team members to develop, personalize, and update treatment plans Ensure all treatment plans and documentation comply with Clarvida policies, state regulations, and accreditation standards Serve as a liaison and advocate between clients, families, community agencies, schools, courts, child welfare agencies, and other involved parties Participate in supervision, treatment team meetings, and case consultations Present client progress and treatment recommendations during team meetings Support colleagues and contribute to positive outcomes for all clients and families served Complete clinical documentation accurately and within established timeframes Participate in peer audits and quality improvement initiatives Utilize data and performance metrics to support the continuous improvement of client and program services Participate in on-call responsibilities as assigned

Requirements

Bachelor's Degree in a Human Services-related field from an accredited college or university Minimum of one (1) year of experience in the mental health or human services field Knowledge of child development and behavior modification principles Ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds Strong written and verbal communication skills Ability to manage complex cases and collaborate with multidisciplinary teams Valid driver's license, reliable transportation, and ability to travel within the community Ability to complete documentation within established timelines

Preferred Qualifications
