1-on-1 Skilled Home Visits (2 hr, 4 hr, 8 hr, 12 hr shifts)
Part-time and/or Per Diem

Your Responsibilities!
Port/PICC line, TPN, IV Pain meds, and Fluids/TPN
Medication Administration, Wound Care, Skilled Nursing services
Provide thoughtful care to client & patients based on personalized Plan of Care
Monitor and track changes in the client/patient’s conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
Proactively updates client/patients’ family on plan of care, health changes, and concerns
