About the role

The Clinical Pharmacist provides expertise in safe, evidence-based drug therapy for high-risk seniors within a multidisciplinary care team. Key duties include conducting comprehensive medication reviews, optimizing treatment outcomes, and leading pharmacy initiatives like deprescribing.

The Job The clinical pharmacist is an essential member of the multidisciplinary care team, providing expertise to promote safe, evidence-based, and cost-effective drug therapy to our members to help advance Scan’s goal of keeping seniors health and independent. In this role, you will be the pharmacist supporting a care team responsible for caring for some of Scan’s highest-risk and most complex members, partnering closely with physicians, advanced practice providers, nurses, social workers, and other care team members in order to dramatically improve patients’ health outcomes. The pharmacist also serves as a trusted clinical leader across the enterprise, helping drive quality, affordability, and positive member outcomes. You Will Work within a multi-disciplinary team to provide collaborative chronic disease management through direct patient care or through consultation with the primary NP and PCP. Complete comprehensive medication reviews high-risk, complex patients, making recommendations to the primary NP and PCP regarding medication changes. Evaluate therapeutic effectiveness, recommend medication adjustments, support deprescribing when appropriate, and help optimize treatment outcomes while minimizing medication burden and risk. Help communicate recommendations regarding medication changes and education on medications to patients. Identify and address barriers to medication access, affordability, and adherence. Collaborate with members, caregivers, providers, pharmacies, and community resources to facilitate timely access to medications and ensure cost-effective therapy options. Support formulary optimization and appropriate utilization of pharmacy benefits. Participate in interdisciplinary case reviews and provide medication-related recommendations for members with complex medical, behavioral, and social needs. Educate colleagues regarding best practices and evidence-based guidelines. Provide training to the clinical staff in association with pharmacy specific initiatives and general clinical training at a regular cadence. Help lead pharmacy initiatives such as for deprescribing or medication adherence. May be responsible for the review and approval of certain Part B medications for members. Ensure pharmacy-related activities comply with applicable federal, state, CMS, accreditation, and organizational requirements. Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks; and participating in professional societies. Your Qualifications Graduate or Advanced Degree, Pharm D required. Active Pharmacist License Required. Must have a CA License or be willing to obtain one. Minimum of two years of strong clinical pharmacy experience (required) Responsible for having a strong clinical background and knowledge of chronic disease states such as diabetes, heart failure, hypertension, Asthma and COPD (required) Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience preferred. Requirements

Requires a PharmD degree, an active Pharmacist License (CA or willingness to obtain), and at least two years of strong clinical pharmacy experience. Knowledge of chronic disease states such as diabetes and heart failure is required, while PGY1/PGY2 residency is preferred.
