The APC provides high-quality care for high-risk patients through comprehensive assessments and the implementation of treatment plans. They collaborate with multidisciplinary teams and monitor patient outcomes to improve clinical practices.
Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care. Participate in interdisciplinary team meetings and case conferences to discuss patient care and treatment plans.
Quality
Improvement: Monitor and evaluate patient outcomes to identify areas for improvement in care delivery. Participate in quality improvement initiatives and research projects to enhance clinical practices and patient outcomes.
Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes. Ensure compliance with all regulatory and documentation standards.
