About the role

The Behavioral Health Care Manager provides clinical care, brief counseling, and treatment coordination in collaboration with primary care providers and psychiatrists. Key duties include performing psychosocial intakes, developing treatment plans, and conducting proactive follow-ups to assess patient response.

This position is being recruited and hired by Sheppard Pratt Solutions [https://www.sheppardpratt.org/solutions/] on behalf of Greater Baltimore Medical Center (GBMC). [https://www.gbmc.org/] As the largest nonprofit provider of behavioral healthcare in the nation, Sheppard Pratt teams with health care systems across the country so they can provide the highest quality acute psychiatric services in their community.

What to expect.

The Behavioral Health Care Manager position is for master’s-level, licensed clinicians. This position will join outpatient teams as a behavioral health care manager with Sheppard Pratt’s Collaborative Care Program at an assigned practice location. The Behavioral Health Care Manager will provide clinical care in collaboration with primary care providers and consulting psychiatrists. Care will consist of brief counseling, coordinating treatment, and providing proactive follow-up to treatment response utilizing a collaborative care approach.

Specific Responsibilities

  • Outreach to patients referred by primary care, discussing the service and scheduling.
  • Perform comprehensive psychosocial intake.
  • Develop, coordinate, and update patients' treatment plans.
  • Provide patients with brief, evidence-based psychotherapy such as cognitive behavioral therapy, motivational interviewing, behavioral activation, and problem-solving treatment.
  • Support medication management, including consultation with psychiatrist.
  • Conduct proactive follow-up to assess and document patient response to treatment.
  • Attend team meetings as required by the Program Manager.
  • Develop working relationships with primary care providers in practice to collaborate about patients and support knowledge of the current state of Collaborative Care services.

What we need from you

  • Master’s Degree in Social Work, Counseling, Mental Health, or related field.
  • Requires a current social work license (LMSW, LCSW-C) or counseling license (LGPC, LCPC) in Maryland at the time of appointment.

The pay range for this position is $65,000 minimum to $83,000 maximum. Pay for this position is determined on a number of factors, including but not limited to, board certifications and years and level of related experience. Care provided in both virtual/telehealth and face-to-face visit formats based on the needs of the client and host practices. Behavioral Health Care Managers may telecommute up to 3 days per week, with 2 days on-site.

Requirements

Candidates must hold a Master's Degree in Social Work, Counseling, Mental Health, or a related field. A current Maryland license as an LMSW, LCSW-C, LGPC, or LCPC is required at the time of appointment.
