Job Overview We are seeking a dedicated PRN Nurses LPN/RN to join our healthcare team. This role offers flexible scheduling to support our residents' needs across various shifts. The ideal candidate will possess a strong background in diverse nursing practices, including critical care, long-term care, and acute medical settings, with a focus on delivering compassionate patient-centered care. Duties As a LPN/RN you will need to perform comprehensive patient assessments, vital sign monitoring, and physical examinations to inform individualized care plans. Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and medical documentation standards. Provide specialized care for residents with dementia, Alzheimer's disease, and memory impairments, including memory and behavioral health management. As a LPN/RN you will Assist with IV infusions, catheterization, tube feeding, Wound Vacs Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and post-acute care services. Ensure infection control procedures are followed diligently to maintain a safe environment for residents and staff. Document all patient interactions thoroughly using EMR/EHR systems such as Point Click Care; maintain clinical confidentiality at all times. Qualifications Qualifications Valid LPN or RN licensure in good standing; additional certifications such as BLS/CPR required. Demonstrated experience working in diverse healthcare environments including hospital medicine, long-term care, assisted living, or skilled nursing facilities. Proficiency with EMR/EHR systems like Point Click Care; familiarity with ICD-9/10 coding is preferred. Strong knowledge of geriatric care principles, dementia care strategies, and acute pain management techniques. Experience working with individuals with developmental disabilities or behavioral health needs is advantageous. Ability to perform venipuncture, sterile procedures, and basic life support confidently. Excellent communication skills for effective caregiver communication and patient assessment; compassionate approach essential.
