POSITION SUMMARY Provides assessment, referral, education, service planning, and care coordination services to CrescentCare clients with a focus on increasing self-management and adherence to behavioral health treatment plans. They will focus on work with persons living with chronic mental health conditions. Works as a part of an interdisciplinary team with CrescentCare and/or community providers. This position will have a focus on initiating and maintaining community behavioral health partner relationships. Maintains a close working relationship with direct supervisor, participates in active supervision, and remains receptive to feedback on professional development to support high quality service provision. KEY DUTIES AND RESPONSIBILITIES Ensures expedient client access to needed behavioral health resources Engage clients in contact either through face-to-face, telephone, text messaging and complete all notes and required documentation, providing a minimum of 24 total documented UOS per day. Administer Biopsychosocial (BPS) assessments to determine accurate level of care as indicated by American Society of Addiction Medicine (ASAM) criteria for patient placement, including SAMHSA GPRA and create an individualized plan for services with each client. Assist clients in articulating personal goals for recovery. SAMHSA case manager will support clients in identifying and creating recovery goals and objectives and work the treatment team in determining the steps he/she needs to take to achieve these goals. Works according to standard with the assigned care team, if applicable. Takes responsibility for the management of assigned caseload using evidence-based interventions. Submits well-written notes based on treatment/care/recovery plans, thorough strengths-based assessments, and other required paperwork within required deadlines. Develop and maintain strong working relationships with and routinely consult with case managers, healthcare providers, psychiatrists, and health educators both within the agency and the community at large; participate in huddles and care team meetings as requested and able. Responsible for ensuring clarity on program expectations and standards, including but not limited to record keeping and documentation, behavioral expectations, work quality and productivity expectations, effective teamwork, and client-centered ethical practice. Responsible for remaining receptive to coaching and/or participating in disciplinary action when expectations and standards are not met. Attends all meetings and trainings as directed by supervisor. Attends a minimum of 20 hours of continuing education annually. Other duties as assigned by supervisors. POSITION REQUIREMENTS The minimum education level required for the position is an MSW. PREFERRED minimum experiential level for the position is one year providing social work services. The minimum required license is a CSW. Minimum age of 21. Must have reliable transportation and a valid driver’s license, no license denials, revocations, or more than one suspension in the past three years valid car insurance no more than one at-fault accident in the past three years no more than two moving violations in the past five years SKILLS & COMPETENCIES Necessary technical skills to use electronic health record, incident reporting system, email, and other information systems. Excellent communication skills and the ability to schedule work production to meet timelines with attention to detail. Knowledge of and the desire to work with a cross-disability population including, but not limited to, those living with HIV and/or mental health issues. Ability to communicate effectively with diverse individuals and groups. Ability to work under pressure in a fast-paced environment. Ability to handle crisis intervention. ESSENTIAL PHYSICAL REQUIREMENTS Travel is required for the purpose of meeting with clients, stakeholders, and off-site personnel/management. Sitting for extended periods of time. Physically able to participate in training sessions, presentations, and meetings. Physically able to provide services in office-based and community-based settings where climbing several flights of stairs might be necessary.
