This is a travel - temporary assignment.

Responsibilities

  • Administer prescribed medications and treatments per physician orders and facility policy.
  • Monitor resident status, assess changes, and report findings to the charge nurse and physician as needed.
  • Provide and document nursing care, including ADLs, vital signs, wound care, and injections as allowed by scope of practice.
  • Collaborate with the interprofessional care team to implement and adjust individualized care plans.
  • Maintain accurate and timely documentation in electronic or paper records, ensuring confidentiality and compliance with regulatory requirements.
  • Respond to resident needs and call lights promptly, promoting safety, comfort, and dignity.

Requirements

  • Active LPN license in Vermont (in good standing)
  • Graduation from an approved practical nursing program (LPN diploma)
  • Current CPR/BLS certification (or ability to obtain)
  • Prior experience in long-term care or skilled nursing settings preferred
  • Ability to work evenings and/or overnight shifts
  • Ability to pass required background check and drug screening
