Responsibilities

  • Deliver coordinated nursing care for a patient or assigned group of patients using the nursing process and established standards.
  • Assess patients, analyze data, and develop and implement a care plan with measurable outcomes.
  • Monitor and collaborate with interdisciplinary team members, supporting safe, healing, and compassionate care.
  • Supervise and direct activities of various levels of nursing staff; delegate duties as appropriate.
  • Participate in discharge planning and provide education to patients and families to support continuity of care.
  • Document care accurately in the patient record (EPIC experience required) and ensure safety and quality in the home hospice setting.

Requirements

  • Active RN license in the state of practice (California)
  • Basic Life Support (BLS) certification; Cardiopulmonary Resuscitation (CPR) required
  • Advanced Cardiovascular Life Support (ACLS) certification required
  • Hospital Fire and Life Safety (HLFS) certification required
  • Minimum 2 years of recent ER nursing experience (acute care) preferred
  • EPIC electronic medical record experience required
  • BSN (Bachelor of Science in Nursing) preferred
  • Ability to work in a home healthcare - hospice setting and coordinate with multidisciplinary teams

Education and Licensing Notes • Licensure

  • Registered Nurse (RN) license in California
  • Certifications: BLS/CPR, ACLS, HLFS; EPIC experience required
