Responsibilities

  • Deliver coordinated nursing care for an assigned group of patients according to established standards of care and the nursing process.
  • Gather and analyze assessment data, identify care issues, and develop a plan with prescribed interventions to achieve outcomes.
  • Implement the plan, coordinate care delivery with other disciplines, and promote a safe, healing environment.
  • Evaluate progress, identify outcomes, and adjust care plans as needed; delegate and supervise duties of the nursing team as appropriate.
  • Collaborate with patients, families, and the healthcare team; provide education and discharge planning to ensure continuity of care.
  • Document care using the appropriate electronic health record system (EPIC) and participate in interdisciplinary rounds and quality improvement activities.

Requirements

  • Current Registered Nurse (RN) license in the state of practice (California preferred for this assignment).
  • Minimum two years of recent nursing experience in the field.
  • Basic Life Support (BLS) or CPR certification from an approved vendor; ACLS required per department policy; HLFS (Hospital Fire & Life Safety) certification required.
  • EPIC EMR experience is required.
  • BSN is preferred.
  • Ability to work in a home healthcare - hospice setting and to travel to patient locations as needed.
  • Ability to work a night shift schedule (7:00 PM–7:00 AM) and meet guaranteed hours.
