As a Mental Health Nurse, you will be a valued member of a multidisciplinary mobile treatment team, providing person-centered, recovery-oriented care in the community. You'll work collaboratively with behavioral health professionals to promote wellness, independence, and long-term recovery while helping individuals achieve their personal health goals.

Key Responsibilities
Serve as a member of a multidisciplinary Assertive Community Treatment (ACT) team.
Conduct comprehensive mental health and nursing assessments.
Assess participants' physical health needs and provide ongoing nursing support.
Coordinate referrals and communication with primary care providers and community physicians.
Administer and monitor medications in collaboration with the ACT psychiatrist.
Provide medication education and support to promote adherence and recovery.
Deliver health education focused on wellness, chronic disease management, and self-management.
Monitor participants for changes in mental and physical health status and respond appropriately.
Participate in treatment planning and coordinate care with internal and external providers.
Maintain accurate, timely, and compliant clinical documentation.
Provide rehabilitation, recovery, and support services in community-based settings.
Promote a culture of inclusion, belonging, dignity, and respect.
Perform other duties as assigned.

Candidates must hold a current, unrestricted New York State Registered Nurse license and possess a valid driver's license. A minimum of one year of nursing experience in a mental or behavioral health setting is preferred.
