Keystone Community Health Services (KCHS) Medical Case Manager Full-Time Monday-Friday, 8:30 am - 5:00 pm Keystone Community Health Services is seeking a Medical Case Manager to join the team.

The Medical Case Manager is responsible for coordination of medical and related services for HIV+ individuals.

The primary focus of the Case Manager is to promote patient adherence with the treatment regimen prescribed by their HIV Provider, including assistance in resolving barriers to HIV primary care.

The Medical Case Manager provides face to face and non-face to face services both in the office and in the community.

Qualifications

Registered Nurse and/ or have a bachelor’s degree in social work, psychology, sociology, or related field, or bachelor’s degree in a non-similar field and two years’ experience in case management, social work, and/or a Community Health Worker Certification.

Bilingual in Spanish, French, or Haitian Creole a plus. Keystone Health is an Equal Opportunity Employer
