About the role

The Home Health Aide provides personal care and assistance with daily living activities to clients in their homes under the direction of licensed staff. They are also responsible for documenting services, performing basic medical tasks, and assisting with telemonitoring equipment.

Scheduled Weekly Hours: 32 Work Shift: Days (United States of America) Job Summary: The Home Health Aide's primary function is to provide personal care to clients and assist with activities of daily living, simple meal preparation, non-sterile wound care, and rehabilitation exercises.

The Home Health Aide provides care under the direction of by the licensed professional staff member.

The Home Health Aide is responsible for documentation of services provided.

The Home Health Aide may assist the agency with the telemonitoring program by installing, removing and cleaning of equipment as the need arises.

Minimum Qualifications

Education: High School diploma or GED. Successful completion of a Nursing Assistant Course with certification.

Experience

  • Work experience in a health care related setting dealing with patients for at least one year is required. Must have completed a Nursing Assistant Program.
  • Licensure/Certification: Current certification as a Nursing Assistant required. Must have a current American Heart Association BLS for Healthcare Provider Card.

  • Knowledge/Skills: Demonstrated knowledge in proper body mechanics, infection control measures,vital sign assessment, and use of durable medical equipment to include hospital beds, wheelchairs, walkers, etc. Knowledge of clerical procedures and medical terminology. Skill in English grammar and spelling. Neat, legible handwriting. Excellent communication skills. Ability to use office equipment. Knowledge of patient care tasks and techniques to provide nurse aide care. Ability to report changes in the patient's condition to the professional nurse. Knowledge of organizational policies and procedures to administer patient care. Knowledge of common safety hazards and precautions to establish a safe working environment. Competency and skills assessment demonstration is required.

Must have a valid South Carolina driver's license with a good driving record. Must have a means of transportation.

Contacts

Patient/family contacts in the home and various community settings. Contacts with other health care professionals, community agencies, and agency personnel.

Demands/Environment: Frequent standing. Frequent walking. Frequent lifting/moving patients. Intermittent lifting, moving or carrying up to 50 lbs. Intermittent pushing or pulling up to 50 lbs. Frequent use of finger/hand dexterity. Frequent reaching with hands/arms. Frequent stooping, kneeling, crouching or crawling. Frequent driving. Corrected hearing and vision to normal range. Exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Requires work under stressful conditions, deadlines, or irregular hours. May be required to take call and/or work weekends/holidays based on the needs of the department. Ability to read and comprehend. Possible exposure to unclean environments, toxic substances, pharmaceutical preparations, hazardous and unsafe driving conditions, and other conditions common to delivery of health care in the community.
