Your Responsibilities: Assists with admission, transfer and discharge of residents as instructed by Charge Nurse.
Assist residents with tending to personal care and activities of daily living.
Provide direct patient care as assigned by Charge Nurse.
Provide proper documentation of resident care.
Promptly communicate with Charge Nurse and leadership team with any changes in the resident’s condition.

High School diploma or GED required.
105 hours Certified Nursing Assistant Program as approved by the Indiana State Board of Health or equivalent as approved by the Indiana State Board of Health.
Requires having name placed on Certified Nursing Assistant Registry at time of hire as required by the Indiana State Board of Health.
Candidates must hold a high school diploma or GED and possess a valid Certified Nursing Assistant certification approved by the Indiana State Board of Health. Applicants must be listed on the Indiana Certified Nursing Assistant Registry and maintain good standing throughout their employment.
