Case Manager I

Participate in multidisciplinary meetings and collaborate with internal teams and community partners to support coordinated service delivery.

Complete timely and accurate case notes, assessments, referrals, and service documentation in HMIS and other required systems.

Complete required trainings and policy acknowledgments and perform other duties as assigned.

Work Schedule

This is a full-time position that combines shelter-based case management, client engagement, resource coordination, housing-focused planning, and participation in multidisciplinary team activities. While the typical schedule is Monday through Friday, occasional schedule flexibility may be required to support client needs, program operations, and organizational priorities.

Compensation & Benefits

The anticipated starting pay range for this role is $22.00 to $25.30 per hour, paid out bi-weekly. Final placement and starting pay rate will be determined based on the candidate's experience, qualifications, and internal equity considerations.
