  • Supervisor: Program Director SUMMARY OF POSITIONAs part of the interdisciplinary GCAOP treatment team, the Case Manager is responsible for guiding clients in the establishment of benefits (Medi-Cal, ACCESS) and assistance in accessing outside medical, dental, financial, housing, transportation, clothing, retail, legal, and immigration resources. The Case Manager will be responsible for collaboratively assessing resources and needs, making referrals and linking to services, and monitoring/following up to ensure that the client and family’s needs are met. The Case Manager monitors clients’ behavior and functioning, assists clients in developing skills to support their overall functioning, and collaborates with the treatment team to ensure quality care for all clients. Additionally, the case manager will be responsible for establishing/maintaining active and effective liaisons with relevant community agencies to foster linkages.

  • Comply with all policy and procedures, including: DMH Manual, LAUSD Requirements, and Gateways Child and Adolescent Outpatient Staff Handbook. Participate in Child and Family Treatment (CFT) team meetings and/or School Meetings Provide services in the community, home, telehealth, or wherever the family feels most comfortable receiving case management services; must be flexible regarding time and place of meetings with client and family. Provide case management linkage services to client and family as identified during CFT and/or School meetings. Assists clients in obtaining California I.D., immigration documentation, and other services as needed Provide rehabilitation services which include assistance in improving, maintaining, or restoring one or more functional skills, daily living skills, social and leisure skills, grooming and personal hygiene skills, and support resources; and/or medication education. Work collaboratively with the multi-disciplinary treatment team. Communicate and interact with client and family from a strength-based, collaborative, and supportive stance. Knowledgeable about government programs, such as SSI, Medi-Cal, etc. Maintain active and effective liaisons with relevant community agencies to foster linkages. Complete all required documentation in a timely manner, as specified in the L.A. County Short Doyle/Medi-Cal Manual, including completing progress notes within 24 hours. Understands billing codes and requirements, including billing and documentation requirements for case management services and socialization improvement/rehab services. Input documentation into the Electronic Health Record system. Attend trainings to maintain technical skills and foster professional development. Attend and participate in weekly individual supervision. On call responsibilities when crises arise Responds appropriately to emergency situations and exercises good judgment by objectively and unemotionally analyzing facts, weighing evidence, reaching sound conclusions, and asking for support as needed. Documents all services provided within program guidelines and expectations in a timely manner. Provides the treatment team with ongoing updates on clients’ functioning, behavior, compliance, progress, and engagement. Performs other additional tasks as directed. ESSENTIAL SKILLS Ability to work and relate effectively with multidisciplinary treatment team, clients, and public. Ability to communicate effectively in English, both verbally and in writing. Ability to remain calm, objective and dependable in situations where clients are emotional or uncooperative, and situations where there are assignments that must be met. Ability to appropriately follow instructions and feedback from supervisors. Functional experience with Los Angeles County Mental Health System. Provide at least 25 hours of service delivery weekly Ability to sit for periods of 2+ hours. Experience with an electronic health record system Knowledge of basic computer skills, including Microsoft office, email communication, video conferencing, and typing. QualificationsEDUCATION &

Candidates must possess a high school diploma with 5 years of experience, or a bachelor's degree in a mental health field, or 2 years of college with 3 years of relevant experience. A valid California driver's license and successful background clearance are required.
