Overview

The RN Case Manager is responsible for direct care and case management of assigned clients, including conducting weekly in-home nursing visits for physical assessments, ensuring home safety, and overseeing medication management. They also collaborate with regional directors and staff to improve services, plan and evaluate care, and act as an on-site clinical resource when needed.
