The Case Manager will provide direct support to program participants, focusing on ensuring engagement with preventative health services and actively removing barriers to accessing care within the SIRHSO program. Responsibilities include managing a caseload, conducting intake, facilitating community education, coordinating with dieticians, and tracking referral outcomes for program reporting.
Candidates must possess a Bachelor’s Degree in Human Services or a related field, or qualify as a Licensed Practical Nurse (LPN) or Mental Health Professional (MHP). Preferred qualifications include a minimum of three years in case management or client support services, along with strong organizational and communication skills.

Job Title: Case Manager
The Case Manager will provide direct support to program participants, ensuring engagement with preventative services and removing barriers to accessing care. Immediate Supervisor: SIRHSO Director
Full-time, 40 hours/week. Afterhours on-call rotation required.
FLSA Classification: Non-exempt
Bachelor’s Degree in Human Services or related field. Qualifies as a Mental Health Professional (MHP); or qualifies as a Licensed Practical Nurse (LPN). Minimum of three years of experience in case management or health related support services preferred. Strong organizational and communication skills, with experience in community outreach preferred.
$44,012 annually.
