• As an RN on the Home Care Team, you will provide services to those living primarily in Cleveland – east, west and the downtown district.
• Further coverage will expand as far west as Vermillion.
• In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort.
• This position offers a flexible schedule, allowing you to maintain a healthy work-life balance, all while changing the lives of your patients and their loved ones every day.
• This is a PRN role.
• Caregivers are required to work 48 hours in a 6 week period.
• Weekend coverage is required one shift per 6 weeks.
• A caregiver who excels in this role will: Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols.
• Implement and monitor patient care plans.
• Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient.
• Perform accurate and complete admission assessments and reassessments within a specified time frame.
• Assess patient for safety risks, abuse and neglect.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Assess and document patients' pain using appropriate pain scale and manage patients' pain then.

Minimum Qualifications

for the ideal future caregiver include: Graduate from an accredited school of Professional Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross Minimum one year current clinical experience as a registered nurse One year of clinical experience as an RN in an acute medical/surgical setting Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage Preferred qualifications for the ideal future caregiver include: Bachelor’s of Science in Nursing (BSN) Cleveland Clinic Home Care Services is a full service home health agency dedicated to continue the World-Class Cleveland Clinic care at home. Requires standing and walking for extensive periods of time.
