The role involves providing direct nursing care according to established organizational policies and procedures, which includes implementing and monitoring patient care plans. The nurse will also serve as a primary coordinator for all disciplines to ensure well-coordinated patient care and manage discharge planning needs.
Candidates must possess a Registered Nurse credential from the Indiana Board of Nursing and have a diploma from an accredited school/college of nursing or required professional licensure upon hire. A Basic Life Support (BLS) Provider certification is required within 30 days of the hire or transfer date.

How you’ll make an impact in this role Execute personalized care plans for renal and diabetic patients, monitoring clinical status to ensure rapid interventions and optimal recovery outcomes. Orchestrate multidisciplinary care teams as the primary clinical coordinator, ensuring a seamless and holistic treatment experience for every patient. Translate complex physician and nursing orders into precise clinical actions, maintaining the highest standards of safety and care integrity. Engineer comprehensive discharge strategies in collaboration with the healthcare team to empower patients for success and reduce readmission rates after they leave our care.
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
