The Pharmacist will evaluate physician medication orders for appropriateness, dosage, interactions, and route, while providing clinical recommendations to physicians and facility staff for medication therapy optimization. Responsibilities also include maintaining patient profiles, ensuring required lab tests are monitored, and collaborating with healthcare professionals to provide support.
Candidates must possess a Bachelor’s Degree in Pharmacy (Pharm D preferred) and ideally have one year of experience in a long-term care setting. A current and valid pharmacist license in the state of practice is mandatory, along with knowledge of drug therapy guidelines and proficiency in EMR/PIS systems.
As a Hospice CNA You will: Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning Monitor & Report: Observe and report changes in the patient’s condition Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use Medication Assistance: Help with self-administered medications per state regulations Emotional Support: Offer companionship and comfort to patients and families. Light Housekeeping: Change linens, assist with meals, and maintain a clean space.
Meets the training requirements of the State Completed a minimum of 75 hours of classroom and supervised practical training 16 hours of classroom must be completed prior to a mandatory 16 hours of practical training Minimum of six months experience including providing personal care At least 18 years of age Ability to read and follow written instructions and document care given Understands hospice philosophy, comfortable providing care to the terminally ill Satisfactory references from previous employers Ability to work with little direct supervision Secure with issues of death/dying Provides a calm manner when in a patient’s home Possess and maintains current CPR certification Demonstrates tact, patience, and good personal hygiene Successfully completes a probationary period This probationary period is to ensure the aide is competent in the above qualifications and is documented by the supervisor and the employee Strong oral and written communication skills Good organizational skills About our Line of Business At Grane Hospice Care, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
