The coordinator is responsible for reviewing prospective admissions against established criteria, policies, and procedures, which includes initiating contact to gather necessary clinical and demographic data. This role also involves scheduling assessments, conducting pre-admission evaluations, and coordinating logistical support like transportation arrangements for patients.
The role requires prompt response to inquiries, assisting prospective patients and their significant others in seeking treatment, and referring inquiries to appropriate external agencies when facility admission is not suitable. Maintaining all documentation related to the admissions process is a key responsibility.
Review prospective admissions against approved admission criteria, policies and procedures.
Initiate contact to gather required clinical and demographic data from patient and other sources.
Respond promptly to inquiry calls.
Schedule assessments.
Conduct pre-admission assessments.
Assist prospective patients and significant others in seeking treatment.
Refer inquiries to other agencies and community resources when not appropriate for facility assistance or admission.
Coordinate with referral sources.
Coordinate transportation arrangements for patients.
Responsible for maintaining all the documentation involved with the admissions process.
