The Occupational Therapist treats referred residents by performing evaluations, setting goals, devising treatment plans, and implementing treatments based on diagnosis and prognosis. Essential duties include maintaining proper medical documentation, supervising assistants, and communicating with the interdisciplinary team regarding patient care.
Candidates must possess knowledge of occupational therapy acquired through an accredited university program and hold a New York State license to practice. Required skills involve effectively gathering information from charts and patients to evaluate, implement procedures, and document treatment effects, alongside strong interpersonal skills for instruction and condition reporting.
Treats residents who have been referred to occupational therapy by the resident’s physician on written order. Identifies residents by diagnosis or observation or staff referral who may benefit from therapy and requests orders for evaluation and treatment.
1. Reviews referrals for occupational therapy and physician’s orders; performs evaluations, sets goals, devises treatment plans, and implements treatments based on diagnosis, resident'’ condition and rehabilitative prognosis. Makes recommendations, sends progress notes, and consults with the physician, as necessary.
2. Maintains proper documentation in medical record including evaluations, progress notes, discharge summaries, and treatment logs. Provides information for care planning, MDS, and quality assurance.
3. Supervises occupational therapist assistants concerning the carrying out of treatment programs.
4. Communicates with interdisciplinary team regarding patient care and consults with families as needed. Acts as an occupational therapy consultant to all departments providing in-services as necessary.
5. Uses effective communication techniques within rehabilitation department and inter-departmentally to maximize team building.
6. Maintains 85% productivity.
7. Other duties as assigned by supervisor.
