The Social Worker plans and participates in social service programs for residents based on their needs, involving contact with community agencies for economic, psychological, and medical assistance. Key duties include organizing the social service program, maintaining an assigned caseload, interviewing residents, and assisting with necessary arrangements for care or relocation.
Candidates must possess either an MSW with healthcare experience, a Bachelor's degree in Social Work accredited by CSWE plus one year of healthcare social work experience, or a Bachelor's degree with two years of healthcare experience and State Education Department certification. Required attributes include imagination, creative ability for a sympathetic approach to illness, a spirit of cooperation, and the ability to exercise independent judgment.
Responsible to: Social Service Director
Plans and participates in social service program for residents in the facility based on their needs, and contacts with community facilities and agencies that can provide economic, psychological and medical assistance.
$21.00 - $27.00 per hour based on experience.
Duties: And responsibilities:
Maintain caseload of 60 residents
Complete assessments.
Initiate and update care plans.
Coordinate team meetings with residents, responsible party, and interdisciplinary team.
Coordinate discharge plan for residents
Assist resident with applications from agencies such as Home Health Care, Department of Social Service, Meals on Wheels, etc. when a resident is discharged and such services are needed.
Participate in in-service education.
Assist Director of Social Service as needed.
Participate in community planning related to the interests of the facility.
Keep abreast of current legislation, trends, resources, facilities related to health field.
Maintain confidentiality
Participate in MDS+ process.
Complete required documentation in resident charts.
Other related duties that may become necessary or as directed by a supervisor, Department Head and/or Administration.
Bachelors degree or higher.
