This role involves providing services, counseling, and support to patients and their families dealing with hospitalization needs, including assistance with transition and post-discharge planning in collaboration with external agencies and staff. The social worker will also collaborate on discharge plans for patients requiring Hospice or inpatient care and develop comprehensive Plans of Care based on patient assessments.
A Master’s Degree in social work is required for practice in California, along with at least one year of experience in an acute or long-term medical setting, though two years is preferred. Essential functions include facilitating discharge plans, determining appropriate Hospice care levels, and developing patient-centered Plans of Care.

Provides services to patients and their families to deal with needs incidental to their hospitalization. Provides counseling, support and assistance in making transition plans. Provides direction for routine aspects of non-medical problems of patients and their families. Assists with the planning and post-discharge of patients with outside agencies, nursing staff, and relatives.
