The Hospital Liaison acts as the primary bridge between hospitals and OhioRISE care coordination teams, focusing on supporting effective discharge planning and ensuring timely transitions of care for youth. This involves connecting families to immediate post-hospitalization services and developing initial safety plans in collaboration with treatment teams.
Candidates must possess an LSW/LPC license along with strong clinical assessment skills and familiarity with behavioral health crisis response. Essential qualifications include exceptional relationship-building abilities, experience collaborating with multidisciplinary teams, and knowledge of community mental health and Medicaid services for youth.

The Hospital Liaison is a key member of the OhioRISE team, serving as a bridge between hospitals, care coordinators, and community resources to ensure youth receive timely, coordinated, and supportive transitions of care. The Hospital Liaison will work directly with hospital partners to support discharge planning, provide immediate linkage to resources after hospitalization, and develop initial safety plans for new OhioRISE members. This role requires strong relationship‑building skills, clinical judgment, and the ability to collaborate across systems.
