About the role

The Speech Therapist will plan and implement specific speech therapy treatment programs for residents across various settings in collaboration with the interdisciplinary team. Key duties include evaluating residents, developing individual treatment plans, delivering prescribed treatments, supervising aides, and maintaining accurate documentation.

Job Summary

Plan and implement specific speech therapy treatment programs for residents in a variety of settings according to accepted Speech Therapy practices and principles and in collaboration with the interdisciplinary team.

Responsibilities

1. Evaluate, develop, and initiate individual speech therapy treatment plan for residents in SNF/OP/Home Health settings.

  • Deliver speech therapy treatments and modalities per physician’s orders.
  • 3.
  • Plan speech therapy services within each patient’s capabilities as indicated.
  • 4.
  • Complete daily schedule of patient treatment with assignment of new residents and/or treatments.
  • 5.
  • Assign and supervise care given by the Speech-Language Pathology Student and/or Restorative Aide 6.
  • Collaborate with physician and nursing staff for plan of care as indicated.
  • 7.
  • Observe and evaluate treatment effects and make recommendations for changes as needed.
  • 8.
  • Timely and accurately record treatment effects in accordance with established procedures.
  • 9.
  • Maintain records pertinent to patient treatments; submit documentation timely.
  • 10.
  • Timely and accurately complete discharge summary.
  • 11.
  • Direct and/or conduct training for residents, staff, and students as directed.
  • 12.
  • Maintain department policies and procedures in regard to safety, infection control and quality improvement programs.
  • Initiate plan of correction based on recommendations from the QAPI Team.
  • 13.
  • Conducts diagnostic speech evaluations and evaluates swallowing difficulties.
  • 14.
  • Recommends therapeutic and functional goals for the residents to help residents reach maximum function.
  • 15.
  • Participate in discharge planning, development and implementation of care plans and resident assessment.
  • 16.
  • Maintain communications with residents and family members on an ongoing basis.
  • 17.
  • Maintain compliance with all Morningside Ministries’ policies, procedures and requirements.
  • Maintain compliance with all federal and state laws.
  • Maintain compliance with all agency regulatory guidelines and requirements.
  • 18.
  • Attend and participate in meetings and trainings as required.
  • 19.
  • Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.
  • 20.
  • Perform general office tasks as required.
  • 21.
  • Perform special assignments, projects, and other duties as required.

Qualifications

1. Requires current certification as a Speech Therapist in the state of Texas. 2. Minimum of one-year related experience preferred. Prior related experience or training in rehabilitative and restorative therapy and experience with aging population preferred.

Requirements

Candidates must possess current certification as a Speech Therapist in the state of Texas. A minimum of one year of related experience, particularly in rehabilitative and restorative therapy with the aging population, is preferred.
