• 1. Assessing the clients readiness for discharge in coordination with the Treatment Team. 2. Making appropriate referrals for placement, case management and aftercare services, as assigned. 3. Coordinating with the Business Office to assure that resident benefits and finance are in order, as assigned. 4. Developing and maintaining an awareness of community resources.
• Assures that clients know and understand their legal status and rights. Tracks legal status on all assigned clients.
• Provides consultation to clients and their families regarding issues arising from the clients' admission, discharge and/or aftercare plan.
• Provide direct treatment for assigned clients and families.

• Team member participation
• Participates as a team member and provides input via reporting observations, concerns and asking appropriate questions.
• Reviews and implements resident treatment plans.
• Actively facilitates and participates in multidisciplinary team meetings and treatment planning meetings.
• Demonstrates knowledge of Professional Assault Response Training (PART) principles and uses PART concepts to prevent and safely manage aggressive behavior.
• Provides leadership in facilitating the team process.
• Provides education/training to other team members as necessary.

• Assessment
• Observes, records and reports resident social and psychiatric behavior.
• Completes the Psychosocial Assessment for assigned clients. This includes obtaining necessary data regarding the clients social and psychological history.
