Position Summary

The Physical Therapist Assistant will implement the plan of care as established by the physical therapist and the physician upholding excellence with quality of care.

Position Responsibilities for a Home Health- PTA

• Identifies and communicates with supervising PT the need for reassessment of patients when necessary.
• Provides treatment in accordance with established standards of practice.
• Conducts individual PT treatments following the established POC under the supervision of the Physical Therapist.
• Demonstrates flexibility in visit scheduling and patient assignments.
• Documents measurable objective data related to patient’s function and accurately assesses patient’s response to treatment.
• Consistently demonstrates sound judgment in planning and implementation of patient therapy programs.
• Communicates in a positive, clear, professional manner with patients, families and other healthcare team members, especially when providing education.

Knowledge, Skills and Abilities

Associate Degree from an accredited physical therapist assistant program. Must meet all certification and licensure requirements as established by the state and national board associations. Must maintain an active license in good standing throughout employment.
