The Case Manager/Mental Health Counselor will maintain a caseload and provide regular case management services to residents. Responsibilities include conducting assessments, developing service plans, and facilitating groups to promote health and independent living skills.
Candidates must have a Master’s Degree in Social Work or a related field, along with a license in Clinical Social Work and at least five years of experience in the mental health field. Experience working with individuals who are homeless or living with HIV/AIDS is also required.

We are seeking a Case Manager/Mental Health Counselor for The Doe Fund's Crystal-Towers Residence. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households.
The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to residents funded by HASA. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.
