The Social Care Navigator provides screenings, service navigation, and case management for eligible Medicaid enrollees concerning their social determinants of health needs, ensuring closed-loop referrals are completed after connecting members with necessary resources.
Candidates must possess an Associate’s degree or equivalent in a human services field and must be bilingual in Spanish and English, along with having basic computer proficiency and a valid driver's license.
$50,000 - $55,000 annually
(Bilingual candidates are eligible for a pay differential)
The Social Care Navigator works independently or as part of a larger team to provide screenings, navigation of services, and case management for eligible Medicaid enrollees as they relate to their social determinants of health needs. The Social Care Navigator will screen Medicaid members for unmet health-related social needs. If members qualify for any additional services, the Social Care Navigator is responsible for navigating & connecting members with such resources. Once connected, the Social Care navigator will communicate with the member to ensure closed loop referrals have been completed.
This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter. You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
