Job description

The Palliative Care Nurse Practitioner provides palliative care consultation visits in the community setting, including home, assisted living and skilled nursing facilities. Palliative care consultation visits may include serious illness conversations, goals of care discussions, advanced care planning/directives, symptom assessment and symptom management recommendations with an emphasis on patient goals and quality of life.
