The RN or LPN Case Manager provides skilled nursing care, patient education, and care coordination to support optimal patient outcomes. They are responsible for assessing patient needs, developing care plans, and supervising Home Health Aides.
The RN or LPN Case Manager provides skilled nursing care, patient education, and care coordination in collaboration with the healthcare team to support optimal patient outcomes. Assess patient and family needs and develop individualized care plans Deliver skilled nursing care and health teaching Monitor patient condition and report significant changes Maintain accurate documentation of visits and care provided Coordinate care with physicians and interdisciplinary team Supervise Home Health Aides and support team collaboration Participate in case coordination, quality improvement, and staff support Assist with staff training, onboarding, and performance evaluation Perform additional duties and leadership responsibilities as assigned What You Offer Graduate of an accredited nursing program Current RN or LPN license in Minnesota Valid driver’s license and auto insurance; ability to travel in all weather conditions Strong clinical, communication, and organizational skills Ability to work independently and maintain professionalism and confidentiality Communicate effectively in English, verbally and in writing. Pass a background study through the Minnesota Department of Human Services.
