This position is eligible for hiring incentives to include sign-on bonus and relocation assistance.
Summary In collaboration with our 4 current physicians and 2 advanced practice providers, you will support our advanced practice providers in their onsite geriatric primary care practice for our long-term care patients by completing the physician required regulatory visits, rounding in our subacute settings, and help support our APPs with MD required services in our assisted living facilities.
This role is supported by nursing and clinic assistants.
This position will provide you with flexibility and independence in your practice while feeling supported by a care team that enables our doctors to have the time to provide outstanding care to our patients.

Schedule Part Time | 0.4 FTE 32 hours every two weeks | You would provide coverage to facilities located in the communities of: Long Prairie, Melrose, Sauk Centre, Paynesville, Cold Spring, Watkins, and Belgrade with shared coverage of our sub-acute facilities in St.
Cloud.

$354,000 - $400,000 per year Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.
