The Care Coordinator will collaborate with children, families, and community partners to connect youth to essential behavioral and developmental health services. They are responsible for managing referrals, maintaining accurate clinical documentation, and conducting community-based visits.
Candidates must possess a high school diploma with 3 years of experience, an associate/bachelor's degree with 2 years of experience, or a master's degree with 1 year of experience. A valid driver's license and reliable transportation are required, with LSW/LPC licensure preferred.
