The Registered Nurse will implement and monitor comprehensive patient care plans while serving as the primary lead to ensure coordinated, seamless care. They are responsible for assessing patient conditions, communicating changes to the medical team, and executing physician orders according to clinical standards.
Candidates must hold a diploma from an accredited nursing school and possess a valid Tennessee or compact state Registered Nurse license. Additionally, a BLS Provider certification from the American Heart Association or American Red Cross is required within 30 days of hire.

How you’ll make an impact in this role Assess patients with chronic or acute heart failure through physical exams, symptom review, health history evaluation and monitor vital signs and weight trends. Educate patients and families on heart failure management, medication adherence, and lifestyle changes Coordinate care plans with providers, including titrating medications per protocols. Manage incoming calls to assess symptoms, adjust care or refer to a physician. Act as a liaison between physicians and patients, facilitating follow-up appointments and diagnostic tests.
