The Nurse Practitioner or Physician Assistant will provide personalized, coordinated health care to patients in nursing homes and assisted living communities. Responsibilities include assessing medical needs, prescribing medications, and implementing treatment protocols to improve patient outcomes and prevent re-hospitalization.
Candidates must hold an APRN or PA license and a degree from an accredited program. A minimum of 3 years of clinical experience in geriatric populations or fee-for-service settings is required.

As one of our advance practice providers, you will exercise your independent judgement to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate. You will work closely with our facility partners and health plan leadership to Implement our Treat in Place Protocols in compliance with all federal and state regulations.
