RN Case Manager - Pediatrics

What you will do as an RN Case Manager

• Manage a patient caseload, including both pediatric and adult hospice patients, completing admissions, assessments, and ongoing visits Develop, implement, and update individualized Plans of Care with the Interdisciplinary Team Provide skilled nursing care including symptom management, wound care, IV therapy, and assessments Deliver developmentally appropriate care and education to pediatric patients and their families Educate patients and families on comfort care, medications, and daily caregiving needs Document accurately and timely while ensuring regulatory compliance Participate in IDT meetings, on-call rotation, and quality improvement activities
• What we are looking for: Graduate of an accredited nursing program with an active Virginia RN license Minimum of 2 years nursing experience (hospice/home health, pediatrics, or acute care preferred) Pediatric experience strongly preferred or willingness to learn and grow in this specialty area Comfort with EMR and technology Reliable transportation with valid driver’s license and insurance Compassionate, mission-driven, and emotionally resilient A heart for mission-driven, relational work with individuals and families

• Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM), Serving patients primarily in the Franklin County area.
