Provide on-call nursing coverage to respond to patient and caregiver crises, deaths, and care needs. Coordinate with physicians and the interdisciplinary team to ensure continuity of care and document all interactions in electronic medical records.
Requires a graduate of an accredited nursing program with a current Virginia RN license and a valid driver's license. A minimum of 2 years of nursing experience in hospice, home health, med/surg, acute care, or geriatric nursing is required.
