RN Case Manager

What you will do as an RN Case Manager

• Provide case management and coordinate care for assigned patients and families Complete comprehensive assessments, admissions, and ongoing evaluations of patient needs Collaborate with the Interdisciplinary Team (IDT) to develop and implement individualized Plans of Care Deliver skilled nursing care including symptom management, IV therapy, wound care, and physical assessments Educate patients and caregivers on medications, pain management, and daily care needs Monitor patient condition and communicate changes to physicians and care team members Maintain accurate, timely documentation in compliance with hospice policies and regulations Provide emotional support and guidance to patients and families Participate in IDT meetings, on-call rotation, and quality improvement activities
• What we are looking for: Graduate of an accredited nursing program with an active Virginia RN license Minimum of 2 years nursing experience (hospice/home health or acute care preferred) Comfort with EMR and technology Reliable transportation with valid driver’s license and insurance Compassionate, mission-driven, and emotionally resilient A heart for mission-driven, relational work with individuals and families

• Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM).
• Starting
• Salary: $75,000/year
