Join our amazing Hospice team as a PRN Social Worker/Counselor, this role perfect for someone seeking a flexible, on-call role to earn extra income while making a meaningful impact.
What you will do
Provide on-call psychosocial support to hospice patients, caregivers, and families by phone and in person.
Respond promptly during assigned on-call shifts and review reports, emails, and messages at shift start.
Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
Support admissions, changes in level of care, and end-of-life needs in collaboration with the after-hours care team.
Complete accurate, timely documentation and shift reports per agency standards.
What we are looking for
Master’s degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
At least three years of social work or counseling experience; hospice or end-of-life care preferred.
Strong skills in crisis intervention, grief support, case management, and critical thinking.
Excellent communication, collaboration, and ability to work independently.
Compassionate, emotionally mature, and mission-driven with a heart for end-of-life care.
Schedule & Compensation
Schedule
PRN (as-needed) position. Team members are required to pick up a minimum of 15-20 shifts per month.
Available shift options include
Weekdays: 5:00 PM – 8:30 AM
Weekends
8:30 AM – 5:00
PM (day) 5:00 PM – 8:30 AM (overnight)
Flexibility to work a mix of weekday and weekend shifts is expected.
