Physical Therapist PRN SNF

• The Physical Therapist PRN in a Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality rehabilitative care to patients recovering from illness, injury, or surgery.
• This position focuses on assessing patients' physical conditions, developing personalized treatment plans, and implementing therapeutic interventions to improve mobility, strength, and overall function.
• The therapist collaborates closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care and optimal recovery outcomes.
• Flexibility and adaptability are essential, as this PRN role requires providing care on an as-needed basis, responding to fluctuating patient census and acuity levels.
• Ultimately, the goal is to enhance patients' independence and quality of life while supporting the facility's commitment to excellence in rehabilitation services.

As a Physical Therapist, you will

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred
