Physical Therapist PRN SNF

• The Physical Therapist PRN at our Skilled Nursing Facility (SNF) in San Augustine, TX, plays a critical role in delivering high-quality rehabilitative care to patients recovering from illness, injury, or surgery.
• This position focuses on assessing patients' physical conditions, developing personalized treatment plans, and implementing therapeutic interventions to improve mobility, reduce pain, and enhance overall functional independence.
• The therapist will collaborate closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care and optimal recovery outcomes.
• As a PRN (pro re nata) role, the therapist will provide flexible, as-needed services, adapting to fluctuating patient census and facility demands.
• Ultimately, this position contributes significantly to improving patients' quality of life and facilitating their safe return to home or community settings.

As a Physical Therapist, you will

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
