The LPN will provide pediatric nursing care, including triage and immunizations, while coordinating medical services for children in foster care and prevention programs. They will conduct home visits, educate families on special health needs, and collaborate with a multidisciplinary team to ensure quality care.
Candidates must have completed an accredited licensed practical nursing program and hold a valid nursing license. A minimum of one year of nursing experience is required, along with computer literacy and proficiency in electronic health records.

In the community Make home visits to children in the Prevention and Foster Care Programs, according to program guidelines Accompany children and families to selected primary care and pediatric and surgical subspecialty visits, as needed In our clinic Triage responsibilities include obtaining and documenting vital signs, collecting specimens, performing vision and hearing screens and administering immunizations for children served in the clinics, as ordered by MD/PNP Care coordination activities Assess, under RN supervision, the medical needs of each child through reviewing medical diagnoses, medical history, and inpatient and outpatient records Provide education for families on how to optimally care for the children in their home settings, particularly around the children's special health care needs, with reinforcement as needed Collaborate within the medical/nursing department and across departments with social workers, education specialists, recreation staff, mental health, and other professionals to best support meeting the child's health care needs Maintain and update plans of care as needed, assuring optimal quality of care Participate actively in multidisciplinary Clinical Care Conferences and Family Team Conferences convened at NAC and selected interagency case conferences held at NAC and in the community Facilitate access to medical, specialty and dental care in the community, seek medical records for all visits attended Other duties: Ensure accurate and timely documentation of all patient care and community-based activities.
